Purchase two Prevacid 24HR, 42 ct on the same register receipt to get up to $29 cash back on your purchase. Make your purchase from January 29, 2012 through February 26, 2012. Submit your original cash register receipt along with this mail in rebate form.

This rebate form states the amount of the rebate will be the “Actual Purchase Price paid for the package.” Purchase Price equals the final amount paid by the consumer after any other discounts or offers and may be less than the maximum rebate amount of $29. Basically, don’t use Extra Bucks or Register Rewards to make this purchase if you want the full rebate amount or purchase this with other items!
